Occupational Therapy Assessments

(Functional Capacity Assessment / Assistive Technology Assessment)

For participants requiring a formal Occupational Therapy assessment and written report.

Before submitting this referral, please ensure:

• A current NDIS plan is in place
• Capacity Building funding is available to cover assessment and report writing time (approx. 12 hours)
• A copy of the current NDIS plan is ready to upload
• Relevant recent reports (if available) are ready to upload

Please provide:

• The reason for assessment (e.g., plan review, change of circumstances, Assistive Technology request)
• Upcoming plan review date (if applicable)
• Any specific questions the assessment needs to address

Assessments include face-to-face time, documentation review, report writing, and liaison as required.

Referrals are reviewed prior to confirming scope, time allocation, and availability.
