Before submitting this form, please confirm:

• A current GP referral for Occupational Therapy has been issued.

• A GP Management Plan (GPMP) and Team Care Arrangement (TCA) are in place.

• You have remaining CDM allied health sessions available in this calendar year.

Fees and rebates

• Occupational Therapy is billed at $190 per hour.

• Medicare rebates are processed after payment; a gap fee applies.
