(Insurance, WorkCover, ICWA, DVA or privately funded services)

For clients not accessing services under NDIS or Medicare CDM.

Before submitting this referral, please ensure:

• The funding pathway is confirmed (private or insurer)
• Written approval has been received from the insurer (if applicable)
• Claim number and case manager details are available (for insurer-funded services)

Please note:

• Occupational Therapy services are billed at $190 per hour
• Where a gazetted or scheme-determined rate applies, the applicable rate will be charged
• Payment terms vary depending on funding source

Services will only be scheduled once funding approval (where required) is confirmed.
